Methods Of Bronchial Drainage: Review

aGöğüs Hastalıkları AD, Kocaeli Üniversitesi Tıp Fakültesi, KOCAELİ For effective airway clearance, ciliary function as well as the content and quantity of mucus must be normal. Excessive or abnormal mucus secretion and structural or functional impairment in cilia cause a secretion-clearance imbalance. Cystic fibrosis, bronchiectasis, asthma, chronic obstructive pulmonary disease (COPD) and respiratory tract infections effect mucociliary clearance and may cause secretion retention, atelectasis and pulmonary dysfunction. In such cases, parallel to the treatment of the primary disease, various methods may be necessary to increase mucociliary clearance. Chest physiotherapy, respiratory devices, and muco-active medications have been used to enhance bronchial drainage. The latter may be divided into four groups: mucolytics, muco-kinetics, expectorants and muco-regulatory agents. Mucolytics show their effect by breaking the sulfide bonds in mucous, muco-kinetics by increasing expiratory flow rate and cough efficacy, expectorants by increasing the mobilization of mucous and muco-regulatory agents by suppressing mucous secretion. In selected patients, clinical benefit may be obtained through the use of appropriate adjunctive treatment methods or combinations.Keywords: Bronchial diseases, physical therapy,expectorantsTurkiye Klinikleri J Med Sci 2005, 25:253-260
