Fetal Cells In Maternal Blood; Feasibility In Non-invasive Prenatal Diagnosis?: Review

Fetal cells circulate in maternal peripheral blood and may be appropriate targets for non-invasive prenatal diagnosis. Circulating cells of fetal origin include trophoblasts, white blood cells, nucleated red blood cells, and hematopoietic stem cells. These cells demonstrate different characteristics and antigenicity when compared to maternal cells and are thus amenable to recovery and analysis. The possibility of analyzing fetal cells recovered from maternal blood could provide high sensitivity and specificity pre-natal screening and diagnostic protocols with no direct risk to the developing fetus. However, the small number of fetal cells in maternal blood (1 fetal cell for 5000 to 108 maternal cells) presents a technical challenge. Sensitive analytical techniques such as polymerase chain reaction (PCR) and fluorescence in situ hybridization (FISH) are essential. Years of investigation have as yet to lead to the development of reliable and consistent protocols. Nonetheless, fetal cell recovery promises to be an attractive addition to non-invasive prenatal diagnosis. This review describes fetal cell types, general strategies of fetal cell isolation, current technical challenges, as well as clinical applications that have been envisioned for the future.
