Left Ventricular Diastolic Function Abnormalities In Behcet’s Disease: A Comparison Of Standard And Tissue Doppler

aKardiyoloji AD, bİmmünoloji-Romatoloji BD, Atatürk Üniversitesi Tıp Fakültesi, ERZURUM Objective: This study aimed to assess left ventricular (LV) diastolic function with conventional Doppler echocardiography (DE) and tissue Doppler imaging (TDI), the latter of which is a new technique in the diagnosis of Behcet’s disease.
Material and Methods: 30 patients with Behcet’s disease and 25 healthy persons were included in this study. All patients and members of the control group were evaluated by M-mode, two-dimensional conventional DE, as well as by TDI. The following conventional Doppler parameters were measured: Peak early diastolic (E) and late diastolic (L) flow velocity, E/A ratio, early diastole flow velocity deceleration time (DT), and isovolumetric relaxation time (IVRT). The variables measured included peak systolic pulmonary venous flow velocity (S), peak diastolic pulmonary venous flow velocity (D), and S/D ratio. The following measurements were derived from the mitral annular velocity by TDI: Early diastolic (Em) and late diastolic (Lm) velocities, and Em/Lm and E/Em ratios.
Results: E values and E/L ratios were lower in Behcet’s disease than those in the control group (p= 0.006 and p= 0.04, respectively). DT values were higher in Behcet’s disease than in the control group (p< 0.001). The S/D ratio was observed to decrease in Behcet’s disease, but not at a statistically significant level (p> 0.05). Em values were found to be lower in Behcet’s disease than in the control group (p= 0.001), and Em/Lm ratios were found to decrease in Behcet’s disease (p= 0.001). Although there was an increase in E/Em ratios in Behcet’s disease in relation to the control group, this increase was not significant statistically (p> 0.05). Individual ratios of E/L and Em/Lm below 1 (p= 0.01 and p< 0.05, respectively) as well as DTs above 240 ms
(p< 0.001) were more prevalent in the Behcet’s disease group.
Conclusion: Measurable impairment of diastolic function was more pronounced in Behcet’s disease when compared with the controls. We conclude that TDI alone or together with conventional Doppler echocardiography is useful in the evaluation of diastolic functional integrity in Behcet’s disease.Keywords: Behcet syndrome, ventricular function, left, tissue echocardiographyTurkiye Klinikleri J Med Sci 2005, 25:190-196
