Evaluation Of Renal Hemodynamics With Furosemide Induced Doppler Ultrasonography In Urinary Obstruction

Renal Doppler Ultrasonography can often be used as a non - invasive method in determination of obstructive and non - obstructive hydronephrosis. However, there is not yet any concensus about the interpretation of the "Resistive Index" (RI) which is an important analytic method of Doppler images. In this study, 18 patients who had obstructive hydronephrosis were evaluated with Doppler examination before and after furosemide injection. Changes in pelvic diameters and the mean value of RI from 3 interlober arteries of each kidney were calculated. Mean pelvic diameter increased from 19.13 mm to 27.06 mm after furosemide injection. Mean RI value also increased from 0.604±0.125 to 0.634±0.135 after injection, however, this was particularly more evident in pediatric age group whereas there were no statistically significant difference in adults. As a conclusion, we think that RI values are increasing in obstructive kidneys with induction of renal diuresis in all patients but especially in pediatric age group and Doppler ultrasonography together with furosemide injection can be used as an effective diagnostic tool in obstructive hydronpehrosis.
