Preoperative Short-term Radiochemotherapy In Locally-advanced Rectal Cancer: Comparison Of Two Different Radiotherapy Fractionation Regimens

aRadyasyon Onkolojisi AD, bGenel Cerrahi AD, GATA, ANKARA Objective: In this study, the effects of differentiated preoperative short-term rectal radiochemotherapy on local pelvic failure, overall survival, treatment-related side effects and sphincter preservation are evaluated.
Material and Methods: 32 rectal cancer patients underwent preoperative radiochemotherapy with concomitant 5-Flurouracil (5-FU) between 1995 and 1999. Patients were divided into two groups of varying fractionation. Group 1, with 18 patients, received a total of 20 Gy external radiotherapy in 4 consecutive days of 5 Gy/fraction and Group 2, with 14 patients, received a total external dose of 25 Gy during 5 consecutive days with 5 Gy/fraction. Both groups underwent surgery within 10 days after completion of radiotherapy. All patients received adjuvant 5-FU and calcium leucovorin chemotherapy postoperatively.
Results: One patient died of ileus just after completion of radiochemotherapy and 1 succumbed to sepsis during the early postoperative period. Overall survival in the median 32 months follow-up (range 1-60 months) for groups 1 and 2 was 39% and 57%, respectively (p= 0.31). Local pelvic failure for groups 1 and 2 was 50% and 36%, respectively (p= 0.43). Varying the fractionation did not result in a statistically significant effect on treatment-related side effects and the frequency of sphincter-sparing surgery.
Conclusion: Variations in preoperative radiochemotherapy fractionation regimens in locally advanced rectal carcinoma did not reveal significant differences in overall survival, local pelvic recurrence, morbidity/mortality and sphincter-sparing surgery.Keywords: Rectal neoplasms, radiotherapy, drug-therapyTurkiye Klinikleri J Med Sci 2005, 25:46-50
