Albumin/creatinine Ratio In Spot Urine: A Screening Test For

aBiyokimya Laboratuvarı, Ankara Numune Eğitim ve Araştırma Hastanesi, ANKARA Objective: Spot urine sampling would appear to be a reliable screening method for the detection of micro-albuminuria in hypertensive patients. Our study aimed to compare micro-albumin measurements alone and the calculation of spot urine albumin/creatinine ratio (ACR) as a prospective screening method in hypertensive patients.
Material and Methods: Forty-four patients were included in this study. Pathologic micro-albuminuria was assumed when the micro-albumin concentration exceeded 30 mg/day in 24 h urine samples. Diagnostic performance is expressed in terms of specificity, sensitivity, positive predictive value (PPV) and negative predictive value (NPV), as well as by area under receiver operating characteristic curve (AUC).
Results: A total of 8 samples (18%) showed pathologic micro-albuminuria in the 24 h urine sample. The diagnostic performance expressed as AUC was 0.96 for micro-albumin measurement alone and 0.93 for ACR. The PPV and NPV were 97% and 63.6% for micro-albumin measurements alone, and 97.3% and 100% for ACR, respectively. Specificity and sensitivity were 87.5% and 88.9% for micro-albumin measurements alone and 87.5%, and 100% for ACR, respectively.
Conclusion: The calculation of ACR in spot urine proved to be of comparable accuracy when compared to micro-albumin determination alone and is more convenient in daily clinical practice.
