Mechanisms And Treatment Options Of Pruritus Associated With Systemic Diseases: A Review

aDermatoloji AD, Kahramanmaraş Sütçü İmam Üniversitesi Tıp Fakültesi, KAHRAMANMARAŞ Itching is not only a main symptom of many dermatological diseases, but one of the symptoms for certain systemic diseases. It negatively affects the quality of life of patients. Almost half of generalized pruritus patients should be examined for the systemic diseases such as renal failure, hepatic cholestasis, polycytemia vera, Hodgkin’s disease, AIDS, hypothyroidism, hyperthyroidism, diabetes mellitus, multiple sclerosis and solid malignancies. Although dermatological examination may appear normal, there may be need for a precise history, complete and careful physical examination as well as detailed laboratory tests for an accurate diagnosis. Psychiatric disorders and drugs should also be considered in pruritus etiology. Besides this, some rarely encountered problems such as notalgia parestatica, brachioradial pruritus, postherpetic pruritus and phantom pruritus should not be ignored. It is thought that dry skin increases the production of some pruritogenic mediators in the elderly. The exact mechanism of the itching observed in uremic and cholestatic patients remains largely known. In these cases, treatment is possible only when the cause is eradicated. It can also be observed that some haematologic diseases may only present with itching. Malnutrition can be another reason behind systemic pruritus. As each of these various diseases are caused by different etiopathogenetic factors, different treatment modalities should be developed. For the treatment of these diseases, agents such as rifampicin, cholestyramine, naltrexone, nifedipine, pentoxifylline, aspirin, thalidomid and methods such as phototherapy and plasma perfusion may be indicated. Understanding these mechanisms, undoubtedly, will provide better and more efficient management schemes for pruritus associated with systemic disorders.Keywords: Pruritus, treatmentTurkiye Klinikleri J Med Sci 2004, 24:664-671
