Interaction Of Serum Leptin Levels And Insulin Resistance

İç Hastalıkları Kliniği, ANKARA Object: The differential property of impaired glucose tolerance and Type 2 Diabetes mellitus is hyperglycemia and insulin resistance. Leptin plays an important role in glucose homeostasis. The aim of this study is to determine the interaction of leptin levels and insulin resistance in postmenopausal women with IGT.
Materials and Methods: Oral glucose tolerance test (OGTT) was applied in postmenopausal women who had 110-126 mg/dl fasting plasma glucose. IGT was determined in 20 patients. During OGTT, insulin and leptin levels were determined. The formulation was made for insulin resistance: Insulin Resistance (HOMA-IR) =[fasting plasma insulin (μU/ml) x fasting plasma glucose (mmol/L)] / 22.5 20 postmenopausal women with normal glucose tolerance (NGT) were included into the control group.
Results: Leptin levels in IGT were lower than leptin levels in NGT (p<0.001). The interaction between HOMA-IR and leptin levels at minute 0 and at minute 120 in IGT group was sıgnificiant (respect to p=0.001, p=0.05). The correlation between at minute 0 insulin and at minute 0 and at minute 120 leptin levels in IGT group was significant (respect to p=0.003, p=0.005).
Conclusion: We determined an independent significant correlation from BMI (body mass index), between leptin and insulin levels. In patients with insulin resistance leptin levels, independently from BMI, were lower than the NGT and lower plasma leptin levels were correlated with impaired glucose tolerance. Our data show that, postprandial and fasting hyperglycemia block the stimulating effect of plasma insulin to the synthesis of leptin.Keywords: Glucose Tolerance Tests, Leptin, Insulin resistance, PostmenopauseTurkiye Klinikleri J Med.
