Nitric Oxide: Physiology And Clinical Importance

Nitric Oxide is a multifunctional biological mediator with diverse roles in the cardiovascular, neurological, immunological and many other systems. Besides its vasodilator effects on vascular smooth muscle cells, it acts as a signalling molecule in endothelial and nerve cells and as a killer molecule by activated immune cells. Agonists and antagonists of nitric oxide have been used in conditions such as importence, septic shock, urinary incontinence, type 1 Diabetes Mellitus. However, inhaled nitric oxide as a specific pulmonary vasodilator has been investigated in detail and it is potentially usefil in the treatment of pulmonary hypertension and ventilation-perfusion mismatch.
Nitric oxide is used succesfully as a selective pulmonary vasodilator in neonates with persistent pulmonary hypertension, children with congenital heart disease, and in adults with pulmonary hypertension or with adult respiratory distress syndrome.
