Combined Oral Contraceptives: Risks And Benefits

Combined oral contraceptives are a mixture of two steroid hormones, a synthetic estrogen and a progestogen. Their primary mechanism of action is to prevent ovulation by suppressing pituitary gonadotropin secretion. They are the most effective means for contraception excluding sterilization and because of effectiveness, reversibility and convenience they are used as an extremely popular method of family planning. It is estimated that oral contraceptives are being used by over 60 million women around the world, therefore their safety for short-term and long-term use is a public health issue of great concern. A large number of epidemiological studies which have become more rigorous, and increased in size and improved in methodology in years have provided an encouraging assessment of oral contraceptives. The bulk of recent data suggest that the newer formulations no longer appear to be associated with an increased risk of myocardial infarction or stroke; they protect against the development of ovarian and endometrial cancers; cancer risk, however seems to be associated with an increased risk both in the early and late fertile years; and the relationship between their use and cervical cancer risk remains to be controversial due to methodological problems. It is generally agreed that when used by healthy women, the noncontraceptive benefits of oral contraceptives outweigh the risks involved.
