Alpha-2 Adrenergic Agonists In Pain Management

Researchers have decided to find out new drugs in controlling pain because opioids develop tolerance and respiratory depression in spite of their strong analgesic activities. The analgesic effects of alpha-2 (a2) adrenergic agonists have been known for many years. These compounds’ potentiating effect of opioid induced analgesia have been shown. Clonidine, which has been available for clinical use for many years, is the most extensively studied drug in this field. It is an a2 adrenergic receptor agonist of the imidazol class with some a1 agonistic pro-perties. Medetomidine and its D-enantiomer dexmedetomidine are more selective a2 agonists. They are introduced for clinical use. Other selective agonists ST91 and azepexole are now being used in animal studies. a2 agonists lower blood pressure, decrease heart rate and improve haemodynamic stability during anaesthesia and surgery. They prevent and stop post-anaesthetic tremor and lengthen the duration of local anaesthetic blockade.
