Isolated Limb Perfusion For Extremity Bone And Soft-tissue Sarcoma

Isolated limb perfusion is a locoregional chemotherapy method which is used especially for the extremity sarcomas and malignant melanomas for limb salvage surgery and adjuvant chemotherapy in operable patients, and for pallation therapy in inoperable patients. THe technique of regional perfusion was an out-growth of cardiopulmonary bypass techniques. Lower limb perfusions are carried out through the common femoral vessel for lesions at or below the mid-thigh level, or through the external iliac vessel for proximal thigh and groin lesions. After the vessels are cannulated they are connected to the heart-lung pump and chemotherapeutic agents are added to system. In this way the drug concentration in the limb is kept constant for an hour without systemic toxicity. In our clinic isolated limb perfusions were carried out in 18 patients, 12 with limb soft-tissue sarcomas and 6 with limb osteosarcomas. The follow up time ranged is between 6-30 months. 12 of them were performed for the purpose of adjuvant or induction therapy and 7 of them are stil alive and free of disease, and 6 of them were performed for the purpose of palliation therapy, one complete and two partial remission was seen.
