Etiopathogenesis Of Nocturnal Worsening In Asthma

Over 80% of people with asthma experience nocturnal attack characterised by cough, wheeze and breathlessness at night. There is a normal circadian variation in pulmonary functions. Asthmatic patients show much greater amplitude in thus rithm. The underlying mechanisms are not yet fully clear but several causative factors for this increased responsiveness at night have been mentioned. Some physiologic variables including sleep, supine posture, airway cooling, changes in epinephrine, cortisol and histamine levels, inhibition of NANC system and coincidental pathogies as well as GOR, sinusitis and sleep apnea are sugested as responsible factors. It has been believed that the common mechanism for these factors is bronchial hyperresponsiveness due to airway inflammation.
