The Diagnostic Value Of Myocardial Perfusion Imaging Method With Stress Thallium-201 Spect In The Coronary Heart Disease

The aim of this study is to investigate the diagnostic value of myocardial perfusion imaging with stres, Thallium-201 SPECT in the diagnosis of coronary heart disease, accepting coronary angiography as the gold Standard. 176 patients were studied, including 119 (66%) men and 57 (32%) women with a mean age of 46±8 years. Exercise Tl-201 SPECT with stres-rest protocol and following within 20 day coronary angiography and left ventriculography were performed in all cases. Accepting coronary angiography as the Standard, the sensitivity, specificity and accuracy of the method of Stres Tl-201 SPECT in predicting coronary artery disease has been calculated in ventricul segments bases. The sensitivity, specificity and accuracy in predicting ventricular ischemia were found as 91%, 53% and 63% for anterior segment; 87%, 56% and 64% for septal segment; 72%, 87% and 83% for apical segment; 83%, 81% and 81% for lateral segment; 100%, 45% and 56% for inferior segment.
False positive results were due to mitral valve prolapse, left bundle branch block, right bundle branch block, muscular bridge, hypertension, diabetes mellitus cardiomyopathy and congenital fistulae of the coronary arteries (LAD) communicating with left ventricul.
In conclusion, it was accepted that the method of myocardial perfusion imaging with stress thallium-201 SPECT has been a valuable diagnostic method in the diagnosis of coronary artery disease and its sensitivity and specificity is higher as compared with exercise electrocardiography. However, particularly in the inferior segment, it was recorded a low specificity caused by false positive results.
