A Case Of Follicular Occlusion Triad

aDermatoloji AD, Trakya Üniversitesi Tıp Fakültesi , EDİRNE Acne conglobata, hidradenitis suppurativa and dissecting cellulitis are diseases that may present together. These three diseases form the “follicular occlusion triad”. If pilonidal sinus is seen in addition to these diseases, the “follicular occlusion tetrad” forms. Some authors have designated these four diseases as the “Acne inversa tetrad”. In the following case of an occurrence of follicular occlusion triad, we determine the clinical features and therapy of the three involved diseases.
A 22 year-old male patient diagnosed with acne conglobata, hidradenitis suppurativa and pilonidal sinus presented with many open and closed comedones as well as cicatricial areas on the face. In addition, several nodulocystic lesions on the neck and nape were observed. Atrophic areas surrounding old fistule mouths were seen bilaterally in the axillae. In the left inguinal area was an abscess measuring 3´3 cm in diameter. Multiple sinus openings were noted in the sacrococcygeal and perianal areas. Although in this case no dissecting cellulitis was remarked, the diagnosis of acne conglobata, hidradenitis suppurativa and pilonidal sinus lead to the resulting evaluation as “follicular occlusion triad”.Keywords: Acne conglobata, hidradenitis suppurativa, pilonidal sinusTurkiye Klinikleri J Med Sci 2004, 24:291-294
