The Treatment Of Complicated Digital Defects By Cross-finger Adipofascial Flap

Objective: The cross-finger adipofascial flap is used in the treatment of dorsal defects of the distal phalanx. The purpose of this study was to investigate the treatment effectiveness of the flap in variously located finger defects as well as in palmar defects.
Material and Methods: Between 1999-2002, cross-finger adipofascial flaps were applied to 11 patients with complicated dorsal and palmar finger defects that were located variously. Pedicle separation was performed on the 10th postoperative day for non-smoking patients. Patients were followed from between 6 months and 3 years.
Results: Cross-finger adipofascial flaps were successful in completely covering the palmar and dorsal finger defects regardless of location. All flaps and grafts healed uneventfully. Joint stiffness was prevented by early flap pedicle separation. None of the patients experienced functional loss in the traumatized fingers.
Conclusion: This flap technique, based upon the utilization of dorsal segmental branches of the digital artery, provided a versatile and reliable cover in the treatment of not only dorsal defects, but of palmar defects as well. The continuity of the digital artery was preserved and functional. Cosmetic sequelae at the donor site did not develop. The major disadvantage of the technique was in its being a two-staged procedure. However, early pedicle separation prevented time and functional loss.
