Parathyroid Hormone Measurements In Two Different Matrices And Temperatures

Objective: The purpose of this study was to investigate parathyroid hormone (PTH) measurements in two types of matrices (serum and plasma) collected and kept under room temperature and cold conditions. Material and Methods: Blood was drawn successively into 4 tubes (2 with EDTA, 2 with clot-activator gel) from 50 patients without parathyroid pathology who attended to our laboratory. Two of the tubes (one with EDTA and one with gel) were kept in cold (embedded in ice) and the other two in room temperature for 15 minutes until centrifugation. Then, the tubes kept in cold were centrifuged at 4°C and the tubes kept in room temperature underwent normal centrifugation to separate the sera and plasma. PTH levels were measured in all sample groups with Immulite 2000 immunoassay on the same day of blood collection. PTH levels outside the reference limits (16-87 pg/mL) were not included in the study and the samples from the remaining 30 subjects were statistically evaluated. Paired t-test was used for evaluating the statistical difference between the following pairs: normal serum-cold serum, normal plasma-cold plasma, normal serum-normal plasma, cold serum-cold plasma. Results: The difference between all pairs were statistically significant.(p= 0.035, p< 0.001, p< 0.001, p< 0.001, respectively). PTH levels were higher in plasma with EDTA than in sera. This was associated with relatively higher levels in cold and lower levels in room temperature. Conclusion: If blood is drawn according to cold chain rules for PTH measurements at blood drawing units or plasma with EDTA is used instead of serum, standardization of this process for the blood sent from the clinics can eliminate the disparity in PTH measurements between outpatients and inpatients.
