Correlation Between Radiographic Stage And Pulmonary Function Tests, Serum Angiotensin Converting Enzyme And Gallium-67 Scintigraphy In Patients With Sarcoidosis

aAtatürk Göğüs Hastalıkları ve Göğüs Cerrahisi Eğitim ve Araştırma Hastanesi, ANKARA Objective: A retrospective study was undertaken to determine the demographic, clinical and laboratory parameters of our patients with sarcoidosis and the relation between radiological stage and serum angiotensin converting enzyme (ACE) level, gallium-67 scintigraphy findings , pulmonary function tests.
Material and Methods: Sixty-one patients with sarcoidosis were retrospectively analyzed
Results: Fourty-one (67.2%) female and 20 (32.8%) male, for a total of 61, patients with sarcoidosis (mean age 44.5±12.4 years) were analyzed. Delaying time (i.e. the time between initial symptoms and hospital admission ) was 560.7±920.7 days. Skin lesions were present in 36.1%, peripheral lymphadenopathy in 26.2% and occular involvement in 6.6% of the patients. As the subject were staged according to chest radiography findings , 1 patient (1.6%) was stage 0, 24 (39.3%) were stage I, 28 (45.9%) were stage II, 6 (9.8%) were stage III and 2 (3.3%) were stage IV. Stages based on computed tomography findings of thorax were as follows; 1 patient (1.7%) was stage 0, 17 (29.3%) were stage I, 31 (53.5%) were stage II, 4 (6.9%) were stage III and 5 (8.6%) were stage IV. Activity was determined in 26 patients (83.8%) of the 31 who had been scanned with gallium-67. Bronchoalveolar lavage cellular profile was analyzed in 10 patients and lymphocytosis was determined in 8. CD4/CD8 ratio was analyzed in 12 patients and the mean was 3.29±3.2. Serum ACE level was high in 60.9% of the patients. Serum ACE level was higher in stage II and III patients when compared to stage I. In pulmonary function tests, FEV1 and FVC, FEV1/FVC, DLCO were all under the predicted level of 80% (namely 50.9%, 29.1%, 65.1%, respectively) in all patients. A statistically significant negative correlation was found between FEV1, FVC, DLCO and the disease stage (p<0.05, r=-0,302).
Conclusion: Whereas computed tomography of thorax is important for staging of sarcoidosis, gallium-67 scintigraphy and serum ACE level were found to have no relation to the progression of disease.
