The Structure And Reliability Of A Self-assessment Scale On Health Care Services

Objective: The importance of primary health care services with regard to public health was first emphasized in the Primary Health Care Conference which was held in Alma-Ata, in 1978. The importance of these services was accepted and primary health care organization was established in 1961 with law. This study was aimed to develop an instrument to measure self-assessment of general practitioners on primary health care services who provide effectively those services; and to analyze the structure and reliability of the measurement instrument. It is expected that the developed scale will contribute to the evaluation and enhancement of undergraduate and postgraduate training programs.
Material and Methods: The research group was consisted of 651 general practitioners who were working in 186 primary health care units in Ankara and agreed to participate in the survey in 2002. Seventy six percent of the general practitioners were working at health centers, 55.5% of them were women and the mean age was found as 35.3 ± 0.3 (24-63) years. A 5-point Likert type scale was developed in the study to assess knowledge levels of general practitioners on their responsibilities who are working at primary health care units. Explanatory factor analyses method was used for the analyses of the data and Cronbach alpha coefficient was calculated for the reliability of the scale.
Results: Due to the factor analyses, the structure of the scale presents 45 items with eight factors namely “child health”, “women health and family planning”, “management”, “environmental health”, “emergency services”, “health education”, “records and forensic medicine” and, “communicable diseases and immunization”. Total variance explained by these eight factors was 69.92% and, the Cronbach alpha coefficient was varying between 0.87-0.92 for each factor.
Conclusion: Limited evidence was obtained for the reliability of the scale in this study. For that reason, repetition of the final version of the scale will provide more healthy/robust evidences for the validity and reliability of the scale. We can calculate every subscale score can be rated separately by dividing the total score obtained from that subscale into the number of items of that subscale. Scores can range between 0 and 5; higher scores indicate higher self-assessment level.
