Exercise-induced Lower Leg Pain: Medical Education

Repetitive stress is usually observed as an etiological factor of exercise-induced lower leg pain. Exercise-induced lower leg pain is a condition frequently encountered in runners and people who regularly exercise. The term shin splint has often been used to define many medical problems inducing lower leg pain. Shin splint accounts for 10-15% of all sports injuries and 60%of lower leg pain suffered by runners. This term usually includes medial tibial stress syndrome (MTSS), tibial stress fracture and compartment syndrome. In this review, MTSS, tibial stress fracture and compartment syndrome were defined in detail and their differences and similarities were explicitly discussed. Currently, exercise-induced lower leg pain is common in active populations and athletes. The incidence of exercise-induced lower leg pain rises, parallel to the increase in the number of both professional and amateur individuals who exercise. Sports medicine specialists are suggested to systematically assess the patient and to take care to make a differential diagnosis. This requires a detailed history, complete physical examination and advanced examination techniques when needed. Physicians should try to avoid the use of the term shin splint. In the light of contemporary knowledge, using the term MTSS to include compartment syndrome and stress fracture diagnoses seems to be the most appropriate choice.
