Brucellosis: Evaluation Of Clinical And Laboratory Findings Of 75 Cases

Enfeksiyon Hastalıkları ve Klinik Mikrobiyoloji AD, Süleyman Demirel Üniversitesi Tıp Fakültesi, ISPARTA Objective: Brucellosis is a common zoonosis in many regions throughout the world, especially in Turkey. Infection is frequently transmitted to humans via discharges of infected animals or unpasteurized dairy products. It may lead to complications regarding different organ systems. The aim of this study was to evaluate the clinical and laboratory findings of patients with brucellosis.
Material and Methods: The clinical and laboratory characteristics of 75 patients with brucellosis treated in our clinic within an 8 year period were investigated retrospectively. The diagnosis of brucellosis was established by one of the following criteria: Isolation of Brucella species in blood or other body fluids or tissue samples, a compatible clinical picture supported by the detection of specific antibodies at significant titers and/or demonstration of at least 4-fold rise in antibody titers in serum specimens taken 2 or 3 weeks apart.
Results: The mean age was 48.8 15.51 years. Of the 75 patients, 62 cases were acute, 11 were subacute and 2 were chronic forms of brucellosis. Overall, 68% of the cases had a history of ingestion of contaminated dairy products. The main symptoms were fever, arthralgia, sweating, and myalgia. Fever, splenomegaly, and hepatomegaly were the most common signs. Hematological involvement was present in 46.6%, hepatitis in 33.3%, osteoarticular involvement in 17.3% and endocarditis in 5.3%. Spleen abscess developed in 3 patients, while orchitis, pneumonia, meningitis and renal abscess developed in 1 each. Serum tube agglutination test was positive in 88% of the cases.
Conclusion: Brucellosis, which is a systemic disease with multiorgan involvement presenting with various clinical signs, is a common problem in our country. It should be considered in patients with chronic signs or symptoms associated with multiorgan involvement, especially in endemic areas.Keywords: Brucella, brucellosis, diagnosisTurkiye Klinikleri J Med Sci 2006, 26:623-629
