Psychiatric Findings In A Patient With Huntington’s Chorea:case Report

Significant features include bizarre grimacing, irregular breathing, disorders of articulation, and arrhythmic and irregular extremity movements. In the early phases, irritability, sadness, antisocial behavior, apathy, emotional disorders, anxiety and psychotic features especially involving persecutory delusions may be seen. In this study, we present the psychiatric picture and the treatment response of a case with Huntington’s chorea.
A 42 year-old male patient, who presented with decreased interpersonal communication and inability to leave the house was admitted for inpatient treatment due to diffuse choreoathetoic extremity movements. The patient indicated that the disorder of ambulation and movements had started 20 years ago and had progressed with time. His mother, his uncle and his aunt also had similar diseases. The patient was diagnosed with Huntington’s chorea according to history, clinical and laboratory findings.
He was started on clozapine therapy and the dose was gradually titrated from 25 mg/day to 300 mg/day. Involuntary movements started to decrease after the dose was increased to 100 mg/day. Risperidone 3 mg/day was added to augment the antipsychotic effect. Verbal communication of the patient increased and psychotic findings decreased. This case shows that atypical antipsychotics may be used to control both involuntary movements and psychotic features in Huntington’s Chorea with psychotic features.Keywords: Huntington disease, psychology, antipsychotic agents, drug therapyTurkiye Klinikleri J Med Sci 2006, 26:569-574
