Adolescent Gynecomastia: Review

Adolescent gynecomastia is usually considered a physiologic condition. Gynecomastia can be detected in up to 70% of boys during puberty. An imbalance in the ratio of estrogen to androgen tissue levels is postulated as a major cause in the development of gynecomastia. This imbalance may be the result of an increase in free plasma estrogens due to an augmented synthesis in the testes or adrenal glands, to enhanced aromatization of precursors or to displacement from sex hormone-binding proteins. Laboratory evaluation should include the measurement of testosterone, estradiol, gonodotropin, and prolactine levels and thyroid function tests.
