Gastroeosophageal Reflux And Importance Of Thoracic Surgery: Review

aGöğüs Cerrahi AD, Gülhane Askeri Tıp Akademisi, ANKARA Gastroeosophageal reflux is a tissue damage or arising symptom due to abnormal backward flow of gastric juice into the esophagus. Although hiatal hernia and gastroeosophageal reflux are both common problems and frequently are associated, each may occur independently from the other. Thus, they should be regarded as separate entities. The aim of the treatment is alleviating symptoms, healing of mucosal lesions and improving life quality by protecting the patients in the future. The long-term medical treatment has limitations on life style and manner. Therefore, surgical treatment option in moderate or severe disease must be considered as a treatment option initially rather than at end-stage disease. Particularly in young patients, medical surgical treatment should seriously be considered due to complications and burden of the disease. The aim of surgical inter-vention is to restore the intraabdominal esophagus and to maintain its narrow gastric entrance. Many different surgical techniques have been employed in treating reflux disease. However, the most widely used surgical procedures for long-term performance of cardia functions is fundoplication right over the gastroeosophageal line. Although a signifi-cant proportion of patients with gastroeosophageal reflux can be surgically managed by either transabdominal or transthoracic approach, there are several well recognized situations in which transthoracic approach yields distinct advantages, such as foreshortened esophagus, nondilatable stric-tures and in redo surgical interventions. The latest advances in treating gastroeosophageal reflux surgically are focused on the use of minimally invasive transabdominal or transthoracic surgical interventions.
Although many strategies exist for treating gastroeosophageal reflux surgically, especially in complicated reflux disease and in a number of special situations in uncomplicated reflux disease, the role of thoracic surgery is undoubtful.Keywords: Gastroeosophageal reflux, fundoplication, hernia, hiatalTurkiye Klinikleri J Med Sci 2006, 26:284-295
