Safe Weaning From ıntraaortic Balloon Pump: Measurement Of Aortic Acceleration And Deceleration Time ındex By Transthoracic Echocardiography

cKardiyoloji Servisi, GATA Haydarpaşa Eğitim Hastanesi, İSTANBUL By examining the postoperative transthoracic echocardiographic (TTE) records, we tried to find out the values of aortic acceleration time (AccTi) and deceleration time (DecTi) indexes to provide a safe separation from intra-aortic balloon pump (IABP) clinically.
Material and Methods: Between January 1998 and December 2003, 4178 patients underwent Coronary artery bypass grafting (CABG) operations in the cardiovascular surgery clinic. The study group consisted of 49 cases. The first group comprised 15 patients who could not be weaned from IABP within postoperative 48 hours in spite of pharmacologic support. The second group was consisted of 34 patients who were separated from IABP within postoperative 48 hours. Fourteen patients who had no need for IABP after CABG were considered the control group without considering age and gender. The study was based on the retrospective examination of the TTE records before weaning from IABP in intensive care unite postoperatively. The AccTi and DecTi indexes of the patients were measured after Apical 5 chamber measurement.
Results: The mean AccTi for both groups were 80 ± 23 msn and the mean DecTi for both groups were 150 ± 60 msn. Overall, the AccTi mean value was 47 ± 35 msn/m2 and the DecTi mean value was 88 ± 43 msn/m2. In the control group, the AccTi mean value was 53 ± 35 msn/m2, and the DecTi mean value was 94 ± 43 msn/m2.
Conclusion: We suggest that to decide for the appropriate time for safe weaning from IABP it may be necessary to measure the aortic AccTi and DecTi, which is a beneficial, noninvasive method and easy to calculate that may find a place in the clinical use.Keywords: Intra-aortic balloon pumping, echocardiography, acceleration, decelerationTurkiye Klinikleri J Med Sci 2006, 26:153-156
