Evaluation Of Metabolic Parameters In Subclinical Hyperthyroidism And The Influence Of Anti-thyroid Treatment

*** Prof.Dr., Kocaeli Üniversitesi Tıp Fakültesi Endokrinoloji ve Metabolizma BD, KOCAELİ Purpose: Subclinical hyperthyroidism is defined as a state in which TSH is low but circulating free thyroid hormone levels are normal. We planned this study to evaluate the metabolic effects of the disease on the peripheral and bone tissues and the influences of the antithyroid treat-ment.
Material and Method: Forty patients with a diagnosis of endogenous sublinical hyperthyroidism and thirty healtly individuals were enrolled as the patient and the control group, respectively. The levels of AST, ALT, ALP, cal-cium, total cholesterol, triglyceride, HDL, VLDL, LDL-cholesterol, ferritine, SHBG were studied in group I (be-fore the treatment), II (control) and III (after the treat-ment). Patients were treated for 4 months. For the pur-pose of statistical meaningful, bone mineral density (BMD) of the femur-neck and L 2-4 were also measured by DEXA only in premenapousal women in all groups. The mean values of TSH, total cholesterol, LDL-cholesterol and BMD of the femur-neck before treatment were significantly lower in the patient group than in the control group (p<0.01). The mean values of ALP, SHBG, were significantly higher in group I than group II (p <0.01). The mean values of TSH, total cholesterol were increased after the treatment. Although the mean values of LDL-cholesterol, BMD of the femur-neck were tended to be increased and SHBG was tended to be de-creased after the treatment, there were no difference between group I and group III. The mean values of ALP, SHBG were significantly higher in group III than in group II.
Conclusion: In conclusion; if there are no progression to overt hyperthyroidism or the evidence of unfavourable cardiac and bone effects, we believe that all patients with endogenous subclinical hyperthyroidism should undergo periodic clinical and laboratory assessments instead of antithyroid treatment.Keywords: Hyperthyroidism , Antithyroid Agents, MetabolismTurkiye Klinikleri J Med Sci 2003, 23:450-458
