Reconstruction Of Orbital Floor Defects With Auricular Conchal Cartilage Grafts

Plastik ve Rekonstrüktif Cerrahi Kliniği, ANKARA Introducton: Orbital fractures resulting from facial trauma are common and they are usually accompanied by orbital floor defects. Bony reconstruction of the orbit is essential for protection of ocular functions and conservation of the aesthetic composition of the face. Various autogenous and alloplastic materials have been proposed for the clo-sure of these defects, all having certain advantages and disadvantages. In this study we present our results with autogenous materials.
Materials-Methods : 16 orbital floor fractures in 12 patients were operated between 1991 and 1999. All patients dis-played periorbital edema where six of them had enoph-thalmos and 8 of them had diplopia. Complex traumas involving the globe have been excluded from the study. Direct X-rays have been performed for all patients while CT scans were reserved for patients having diplopia. All patients were operated within the first two weeks fol-lowing the trauma except for two patients. In all patients a subcilliary incision was performed and then a musculo-cutaneous flap of the lower eyelid was elevated to expose the inferior orbital rim and the orbital floor. The orbital floor defects have been covered using con-chal cartilage grafts. Any accompanying facial fractures have been reduced and fixed using mini and micro plat-ing systems. The patients have been followed up for eight months in avarage.
Results: One of the patients displayed persistent vertical diplopia together with infection (8.3 %) in late postop-erative period. Also another patient displayed vertical di-plopia making a total of two patients out of 12 (16.6 %). No other complications have been observed and the re-sults have been successfull both in terms of function and aesthetics.Keywords: Orbital Fractures, Transplants, CartilageTurkiye Klinikleri J Med Sci 2003, 23:442-449
