Histopathologic Analysis Of Internal Thoracic Artery In Patients With Chronic Renal Failure

aKalp ve Damar Cerrahisi AD, Gazi Üniversitesi Tıp Fakültesi, ANKARA Objective: Internal thoracic artery so far, has been the most frequently used graft in coronary by-pass grafting. Chronic disorders have negative effects on vascular structures. The aim of this study was to detect chronic vascular changes histopathologically in internal thoracic arteries in patients with dialysis dependent chronic renal failure who underwent coronary artery by-pass grafting due to coronary artery disease.
Material and Methods: A total of 23 patients, 11 dialysis dependant chronic renal failure and 12 controls, were studied with respect to intimal thickness, intima-media ratio and medial calcification (%) in the internal thoracic arteries using morphometric techniques. Distal parts of the internal thoracic arteries were fixed. After dehydration with ethanol, samples were treated with paraffine and stained with hematoxylane and eosin dyes. Following eosin treatment, samples were stained with von Kossa technique in order to detect calcification in arterial medial substructures. Results were evaluated with light microscopy using special programs.
Results: Calcification was slightly higher in the medial layer of the internal thoracic artery in patients with chronic renal failure compared to the control group, but this difference was not significant (6.67% vs 6.37%). Neither intimal thickness, nor intima-media ratio showed significant difference between the two groups (72.8 Mm vs 72.2 Mm/0.45 vs 0.47).
Conclusion: No significant differences were observed among groups with regard to intimal thickness, intima media ratio and medial calcification rates. Consequently, internal thoracic artery is a suitable conduit even in patients with chronic renal failure.Keywords: Mammary arteries, coronary artery bypass, kidney failureTurkiye Klinikleri J Med Sci 2005, 25:795-799
