Histological Evaluation Of Liver And Spleen To Determine The Efficacy Of Caspofungin And Combination Of Caspofungin With Mer Penem In Candida Albicans Infected Mice

The aim of this study was to determine the histological changes in the liver and spleen in murine systemic candidiasis and to evaluate histologicaly the efficacy of caspofungin and the combination of caspofungin plus meropenem in the treatment.
Material and Methods: BALB/c mice were infected intravenously with 2 x 106 cfu of C. albicans. At 24 hours postinfection, intraperitoneal therapy was initiated and continued for 7 days. Therapy groups included those given caspofungin 0.5 mg/kg, 1.25 mg/kg, or 5 mg/kg/day (once daily) and meropenem 20 mg/kg/day (twice daily) in combination with caspofungin. On day 8, livers and spleens were removed and after the routine processes, histological changes were examined.
Results: Lymphocytic infiltration around the vessels and cystic channels due to C. albicans infection were present at the portal areas of the liver preparations. This infiltration decreased with high dose caspofungin. In the combined treatment group, liver destruction increased parallel to the increasing caspofungin dose. Generalized fibrosis, relative decrease of and irregularity in the white pulp, and large number of giant cells due to C. albicans infection were noted in spleen preparations. While histological improvement was restricted in caspofungin groups, fibrosis was lacking despite the presence of giant cells in the combination groups.
Conclusion: In systemic candidiasis, high dose caspofungin and meropenem combination therapy was effective in acute splenitis but caused serious liver destruction.Keywords: Candida albicans, liver, spleen, caspofungin, meropenemTurkiye Klinikleri J Med Sci 2005, 25:763-769
