Rhino-orbitocerebral Mucormycosis: A Case Report

Nöroloji Kliniği, Bakırköy Ruh ve Sinir Hastalıkları Eğitim ve Araştırma Hastanesi, İSTANBUL Rhino-orbitocerebral mucormycosis is an unusual fatal fungal disease that is directly spreaded from the infected orbita or sinuses and effects CNS (central nervous system). The active fungoid is found at human’s nasal and pharynx floras and in the earth, rotten bread and fruits as saprofites. The infection is usually seen initially in diabetes mellitus; immunsupresive treatment, hematological malignancies (aplastic anemia, leukemia, myeloblastic syndroms) and after diffused burns. Clinical presentation is as proptosis, total external and internal ophtalmoplegia and early blindness. The mortal property of the disease can be reduced by controling the predisposed systemic diseases, treatment of Amphoterisin B and surgical wound cleaning.
47 years-old male patient who has periferic facial palsy and is given corticosteroid therapy without learning the diabetic history; applied to our policlinic with the complaint of severe headache and proptosis at his right eyelid. Right 2., 3., 4., 5., and 6. cranial nerves patologies were observed at his neurological examination. Laboratory findings show leucositosis and there was inflamatory signal increase and the contrast substance spreads all the maxillary sinus, infratemporal fossa, pterygoid muscle, right of the nasopharynx, sphenoid bone’s major wing’s side, right sinus cavernosus and the skin at cranial and orbital MRI. We took biopsy material from the palatinum durum and the result was pathologically spesific for the fungal infection. The patient was treated with Amfotericin B and surgery was applied to clean the cavity. We tried to emphasize the importance of early diagnosis and treatment at the prognosis of mucormycosis.Keywords: Mucormycosis, prognosis, orbital diseasesTurkiye Klinikleri J Med Sci 2005, 25:745-748
