Primary Intramuscular Hydatic Disease Localized In A Lower Extrem-ity: A Case Report

aGenel Cerrahi Servisi, GATA Haydarpaşa Eğitim Hastanesi, İSTANBUL Although cyst hydatid disease is common in our country, a primary intramuscular cystic lesion localized in the femoral region is a rare entity. The aim of this case report of a hydatid cyst localized in a rare position should be considered in the differential diagnosis of masses located in this area. We provide as well a literature review.
A twenty-one year old male presenting with a lesion in the femoral region causing severe pain and movement limitation was admitted. On exam, a mass measuring 12 x 10 centimeters in size was palpated. The lesion was fluctuant, soft and tender, and was subsequently revealed through computerized tomography to possess cystic characteristics. A pus-like substance was revealed during needle aspiration. Drainage under regional anesthesia was employed. At that time, scoleces and a cyst lining were noticed in the cavity. The patient was taken to the operating room and a partial cystectomy and drainage were performed. The patient was begun on albendazole (800 mg/day) in cyclic use, on which he was maintained for four months after surgery. No further anomalies were observed in whole blood parameters. The patient was instructed to remain on albendazole for another 4 months.
This mass of hydatid cyst in an atypical localisation was evaluated by ultrasonography (US) and computerized tomography (CT). The treatment modality is the same as that for the more commonly seen lesion occurring in liver. Surgical procedures such as pericystectomy and drainage, as well as subsequent medical treatment with albendazole are effective in eradicating the lesion.Keywords: Cysts, extremities,albendazoleTurkiye Klinikleri J Med Sci 2005, 25:593-596
