Neuroacanthocytosis: A Case Report

Neuroacanthocytosis is a progressive disease with a wide range of symptoms. Movement disorder with acanthocytosis (over 3% in peripheral blood smear) is the main clinical presentation. We present a case report of a patient who was initially admitted to our psychiatry clinic with the diagnosis of chronic psychotic disorder and tardive dyskinesia.
Detailed history of the patient revealed that symptoms began with orofacial dyskinesia and later progressed to choreatiform movements in extremeties and trunk, dysarthria, and dysphagia in addition to muscular atrophy, weakening of deep tendon reflexes and generalized tonic-clonic convulsions. Fresh blood smears contained between 25 and 30% acanthocytes and neuroacanthocytosis was considered to be the most suitable diagnosis.
In presenting this case, we emphasize that neurodegenerative disorders may present with psychiatric symptoms from the outset of disease and that this situation can be the source of diagnostic inaccuracies that can be overcome only by the taking of a detailed history.Keywords: Choreatic disorders, movement disorders,psychotic disorder, epilepsyTurkiye Klinikleri J Med Sci 2005, 25:576-580
