Comparison Of Local Corticosteroid Injection And Conventional Physical Therapy In Management Of The Painful Shoulder

There are, however, discrepancies in the effectiveness of conservative treatment methods used in its management. The aim of this study was to compare the efficacy of conventional physical therapy to that of local corticosteroid injection in the treatment of patients with shoulder pain.
Material and Methods: A total of 101 female patents were enrolled in the study and randomized into two groups. Conventional physical therapy, consisting of infrared, ultrasound and transcutaneous electrical nerve stimulation, was administered to the first group. The second group underwent injection of a corticosteroid (methylprednisolone) with a local analgesic. Pain was evaluated by the Visual Analogue Scale at rest, at night, and during passive and active motion. Disability was assessed through the use of a Health Assessment Questionnaire and the Shoulder Pain and Disability Index. Pain and disability measurements were carried out on four occasions: before treatment, and subsequently in the first week, and during the first and third months thereafter.
Results: Forty-two patients (79.2%) in the physical therapy group and 46 (95.8%) patients in the local corticosteroid group completed the three-month study period. The mean age of the patients was 52.56 ± 8.89 years and the mean disease duration was 15.52 ± 26.54 months. When repetitive pain and disability measurements within the groups were compared to each other, there was a statistical difference (p< 0.0001). In both groups, scores obtained before treatment were higher than those obtained in the three post-treatment assessments. There was no statistical difference between the groups in neither pain nor disability in the third month post-treatment (p> 0.05).
Conclusion: In our study, there was significant improvement both with conventional physical therapy and local corticosteroid management in terms of pain and disability at the first week, and after the first and third months post-treatment, as compared with pre-treatment levels. However, when control values taken during the third month after both treatment modalities were compared, no statistical difference between the groups was found. The results from this study showed that although both treatments were effective in the management of the painful shoulder, by the end of the third month post-treatment, neither of the methods was superior.Keywords: Shoulder pain, physical therapy, adrenal cortex hormonesTurkiye Klinikleri J Med Sci 2005, 25:506-512
