C-erbb-2 Gene Expression In Differentiated Thyroid Carcinomas

aPatoloji Bölümü, Ankara Numune Eğitim ve Araştırma Hastanesi, ANKARA Objective: The role of c-erbB-2 oncogene in the pathogenesis of thyroid tumors remains unclear. However, some immunohistochemical studies have suggested that increased expression of c-erbB-2 is a typical feature of papillary thyroid carcinomas, while in others no significant difference was detected within groups of differentiated carcinomas. Our purpose was to investigate the expression of c-erbB-2 in differentiated thyroid carcinomas and to determine differences in c-erbB-2 expression profiles exist between groups.
Material and Methods: Thirty-four papillary carcinomas, 23 papillary micro-carcinomas, 4 widely invasive and 14 minimally invasive follicular carcinomas were included. The estimated fraction of tumor cells manifested by cytoplasmic immunostaining was scored as follows: 0 (no staining), 1 (less than 10%), 2 (10-50%), 3 (50-80%), 4 (more than 80%). Staining intensity was evaluated from 1 to 3: 1 (weak), 2 (moderate), 3 (strong). The distribution of positively stained cells within tumor tissue was qualified as homogeneous or heterogeneous.
Results: Seventeen of the papillary carcinomas and 13 of the papillary micro-carcinomas evinced diffuse staining patterns (score 3-4), with moderate to strong intensity and heterogeneous distribution within the tumors. Staining was also diffuse in all widely invasive and in 4 of the minimally invasive carcinomas. Among these cases, the intensity of staining varied from weak to moderate and homogeneous in distribution.
Conclusion: All tumors included in this study exhibited c-erbB-2 expression. However, papillary carcinomas were found to have more positive and intensely stained cells than follicular carcinomas. This finding suggests a biologically significant mechanism involving c-erbB-2 expression in papillary carcinomas independent of tumor differentiation.Keywords: Thyroid neoplasms, c-erbB-2Turkiye Klinikleri J Med Sci 2005, 25:489-495
