Evaluation Of The Activity Of Intravesical Mitomycin C By Using A Marker Lesion Model In Superficial Bladder Cancer

The recurrence rate of bladder cancer is 20-90% and correlated with stage, grade, multifocality and tumor size. Chemo and immunotherapeutic agents are used intravesically to prevent recurrence and progression. Instillation with Mitomycin C (MMC) after TUR is able to reduce the recurrence rate by 50%.
Patients and Methods: 42 patients with superficial bladder tumor those determined on diagnostic cystoscopy, with a lession primer or recurrent, solitary or multifocal and smaller than 2 cm in largest diameter were included in the study. After performing a cold-cup biopsy from the well-defined lesion we applied 40 mg MMC diluted in 50 ml distilled water intravesically for 2 hours. 10 days after all patients were performed TUR. The ablative effect of MMC was estimated by video recordings and pathologically. Responses were rated as follows: response, no response and progression.
Results: All cold-cup biopsy specimens were reported as transitional cell carcinoma. After administiration of intravesical MMC response was determined in 24 patients (%57). In 16 patients no changes appeared after chemoresection (%38). Progression was observed in 2 patients (%5).
Conclusion: In this study we investigated the ablative effect of MMC. We determined this effect with a well-defined marker lesion model. Administiration of intravesical MMC has an antitumoral activity.
