Assessment Of Surrenal Functions In Patientswith Chronic Renal Failure

****Prof.Dr., Kocaeli Üniversitesi Tıp Fakültesi İç Hastalıkları AD, Neforolji BD, Öğr.Üy., KOCAELİ Background: In the literature the test results related to hypo-thalamus-hypophysis-surrenal axis in patients with chronic renal failure (CRF) are different from each other. In this study, we aimed to study diurnal rythm of adreno-corticotropin hormone (ACTH) and cortisole in patients with CRF and the effect of hemodialysis on them.
Material and Method: Twenty predialysis, 22 hemodialysis and 18 healthy subject as control group were enrolled into this study. All subjects had no Cushing disease or syndrome. All cases were hospitalized and blood sam-ples for ACTH and cortisole were withdrawn at 11 pm and 06 am, in addition blood samples were withdrawn after dialysis period in hemodialysis patients. ACTH and cortisole levels were measured by immunoassay.
Results: Mean serum ACTH levels for predialysis, hemodi-alysis and control group were at 11 pm, 34.3±14.5pg/ml, 32.6±13.6pg/ml, 24.7±11.9pg/ml and at 06 am 31.8±11.7 pg/ml, 29.4±10.3pg/ml, 21.6±9.8pg/ml, respectively. Mean serum cortisole levels for predialysis, hemodialy-sis and control group were at 11 pm, 10.3±3.9mcg/dl, 9.4±3.6mcg/dl, 7.3±2.6mcg/dl and at 06 am, 20.7±5.9 mcg/dl, 19.4±5.8mcg/dl, 14.8±6.3mcg/dl, respectively. In hemodiaysis patients mean serum ACTH and cortisole levels were 28.9±11.3pg/ml, 14.6±4.8mcg/dl, respec-tively. Both 11 pm and 06 am ACTH and cortisole levels were found higher in predialysis and hemodialysis pa-tients than in control group (p<0.05). Mean cortisole level in hemodialysis patients was found lower than mean level of 06 am. Diurnal rythm disturbance were not observed in any group.
Conclusion: In spite of higher levels of mean serum ACTH and cortisole in predialysis and hemodialysis patients than in control group, no diurnal rythm alteration was observed.Keywords: Chronic renal failure, Diurnal rhythm, HemodialysisTurkiye Klinikleri J Med Sci 2003, 23:300-304
