Retrospective Analysis Of The Nerve Repairs In The Upper Extremity:clinical Study

Plastik ve Rekonstrüktif Cerrahi Kliniği, ANKARA Purpose: Peripheral nerve injuries often accompany to the upper extremity injuries because of the anatomical localisation. Assessment of the whole hand function depends on both motor and sensibility functions of the hand. Skillful application of microsurgery in nerve repair techniques is essential in obtaining successful outcomes. The purpose of the study is to evaluate the results of the nerve repair techniques, the epineural, the group fascicular, primary suturation, secondary suturation and the nerve grafting, which were performed in our clinic for the patients with upper extremity injuries, by using the constant two point discrimination test and EMG.Material and Methods: 54 patients who applied with the upper extremity injury to Ankara Numune Training and Research Hospital II. Plastic and Reconstructive Surgery Clinic are evaluated.31 median, 16 ulnar, 3 radial, 28 digital nerve injuries, with a total of 78 nerve injuries in 54 patients, were repaired by the epineural and the group fascicular techniques. Primary suturation in 15, late primary suturation in 48, secondary repair in 6 and the nerve grafting in 9 patients were carried out for the repair. For the follow-ups of the patients, the constant two point discrimination test and EMG were used. Results: The mean age of the 54 patients, 6 female and 48 male, were 26.1. Glass injury history was obtained mostly in the etiology. In 26 median nerve and 14 ulnar nerve injuries treated with the group fascicular nerve repair technique, 50% and 43.8%‘good regeneration findings’ were obtained by EMG, respectively. On the other hand, there were no ‘good regeneration findings’ in 5 median and 2 ulnar nerve injuries, treated with the epineural nerve repair technique. The constant two-point discrimination test was used to assess the sensibility of the hand and 80% ‘perfect and good’ results were obtained in 15 patients in whom the primary repair was performed. Only 30% ‘perfect and good’ results were obtained in 9 patients treated with the nerve grafts.Hematoma formation in 3 and infection in 2 patients were encountered. 2 patients, whose Tinel’s signs were negative at the end of 3 months, were reoperated.Conclusion: We obtained successful outcomes of the group fascicular and the primary nerve repair techniques in this study.Keywords: Peripheral Nerves, Injuries, Upper ExtremityTurkiye Klinikleri J Med Sci 2003, 23:185-188
