First And Second Trimester Down’s Syndrome Prenatal Screening:a Comparison

** Doç.Dr., GATA Tıp Fakültesi, Tıbbi Biyoloji AD, ANKARA Down’s syndrome is one of the leading reason of hereditary mental and motor retardation and the frequency of trisomy 21 which represents the vast majority of affected individuals is 1 in 700 live birth. The prenatal diagnosis is important because of the frequency of this syndrome and the social problems they face after-birth. The risk of having a child with trisomy increases with maternal age. Efforts have been made to identify those at risk of carrying affected fetuses by means of karyotyping. Several biochemical measurements, such as alpha-feto protein, unconjugated estriol and human chorionic gonadothrophin found to be correlated with accurance of trisomy. Low alpha-feto protein and estriol levels and a high human chorionic gonadotrophin level in maternal serum re associated with fetuses of Down’s syndrome. In recent years, it has been suggested that first trimester screening may have a higher detection efficiency than second trimester screening. A combination of nuchal translucency measurement and maternal serum markers (pregnancy associated plasma protein A and free β- human chorionic gonadotrophin) could result in significant improvement in Down’s syndrome detection. However, first trimester screening is not yet routine. Most reports are retrospective and it has not been used widely in clinical practice. The markers in first trimestre screening also have time limitation that one can only perform the tests in certain time of the pregnancy.
