The Effect Of Preoperative Glaucoma On The Prognosis Of Penetrating Keratoplasty

***** Op.Dr., SB Ankara Eğitim ve Araştırma Hastanesi Göz Kliniği, Şef, ANKARA Purpose: The purpose of this retrospective study was to evaluate the effect of preoperative glaucoma history on graft survival after penetrating keratoplasty (PKP). Materials and Methods: Preoperative glaucoma history was defined by glaucoma surgery and/ or glaucoma medication prior to penetrating keratoplasty. According to these criteria, in 29 penetrating keratoplasties there was glaucoma history (Group I) and in 847 penetrating keratoplasties there wasn’t any glaucoma history (Group II). Keratoplasties were performed only if glaucoma seemed to be controlled preoperatively. Statistical analysis was performed by using the Fisher’s exact test to compare the rates of immune rejection and graft survival between groups. Results: With a glaucoma history, clear graft rate was 58.6%, in contrast to 78.3% without glaucoma history (t=5.15, p<0.05). There was no statistically significant difference between the groups with respect to immune reactions (t=0.29, p>0.05). But the rate of graft failure following immune reaction was higher in patients with glaucoma (45.5%) than in patients without glaucoma (18.1%). This difference was statistically significant (t=3.46, p<0.05). Conclusion: Penetrating keratoplasty in patients with glaucoma history have poorer prognosis than those in patients without such a history.
