The Relation Between Subperiosteal Resorption Signs And Serum Parathyroid Hormon And Bone Alkaline Phosphatase Levels In Predialysis Patients

** Dr., Hacettepe Üniversitesi Tıp Fakültesi Radyoloji AD, ANKARA Purpose: Although hand X-Ray graphy has limited sensitivity for detecting subperiosteal resorption in the early stages of chronic renal failure, it is widely in use with biochemical parameters which show osteoblastic activity. In this study the frequency of subperiosteal resorption and its relation with intact parathyroid hormone (iPTH) and bone alkaline phosphatase (b-ALP) levels. Materials and Methods: Hand X-rays graphy, intact parathyroid (iPTH) and bone alkaline phosphatase (b-ALP) levels have been studied in 50 predialysis chronic renal failure patients. Result: Subperiosteal resorption was found in 14% of the patients (five patients had grade I, the others had grade II). Plasma intact parathyroid (iPTH) and bone alkaline phosphatase (b-ALP) levels were significantly higher in patients with subperiosteal resorption than the those having normal X-rays.Conclusion: Secondary hyperparathyroidism which is a frequent complication of chronic renal failure starts in the predialysis period and its frequency increased with the duration of chronic renal failure. Although hand X-rays graphy has a weak sensivity in the early stages, we think when it is use with intact parathyroid (iPTH) and bone alkaline phosphatase (b-ALP) values it may be helpful for early diagnosis and management of secondery hyperparathroid bone disease.Keywords: Hyperparathyroidism, Secondary, Alkaline Phosphatase, Parathyroid HormoneTurkiye Klinikleri J Med Sci 2002, 22:549-552
