The patient was noticed to experience chest pain, palpitation; and develop ventricular bigeminy that lasted for approximately 20 minutes after micturition. Urinary 24-hour vanillymandelic acid level was found to be increased; and magnetic resonance imaging disclosed two masses in right adrenal mass and urinary bladder. The masses were removed. The pathological examination disclosed paraganglioma and adrenal hyperplasia for the bladder and the adrenal masses, respectively. Coronary angiography disclosed normal coronary arteries. This case mimicking an acute coronary syndrome and developing ventricular bigeminy during the attacks represents the wide variety clinical pictures in pheochromocytoma. Keywords: Pheochromocytoma, Bladder, Angina, UnstableTurkiye Klinikleri J Med Sci 2002, 22:59-62
