Varicocelectomy:evaluation Of Our Results With Antegrade Sclerotherapy

**** Arş.Gör.Dr.Gaziantep Üniversitesi, Tıp Fakültesi, Üroloji AD, GAZİANTEP Aim: To determine the place of antegrade sclerotherapy of the internal spermatic vein in the surgical management of clinically significant varicocele together with the success/complication rates during long term follow-up, a prospective study has been carried out. Again the results of the treatment and its effects on spermiogram parameters have been comparatively evaluated before and after the surgery. Material and Methods: From 1996 to 2001, 112 patients with clinically significant varicocele were enrolled in this study program and sclerotherapy of the internal spermatic vein has been performed in all patients. Preoperative evaluation included detailed anamnesis, physical examination, sperm analysis and colour flow doppler ultrasonography in all patients. However, hormone profile and morphologic evaluation of spermatozoa were also done when needed. Durind antegrade sclerotherapy of internal spermatic vein, an incison on scrotoinguinal region under local anaesthesia (Bupivacaine 5 ml., 2%) has been performed (described by Tauber et al., 1988) and 2 ml Aethoxysclerol with a concentration of 3% has been injected for sclerotherapy. Mann Whitney-U test were used for statistical significance.Results: Our results demonstrated a success rate of 81% in patients with left side varicocele (47/58), 88.8% in patients with right side varicocele (8/9) and 88.8% in that of bilateral varicocele (40/45). The recurrence rate was 32% (19/58) on left side and 9.2% (5/54) on the right side (including bilateral cases). Recurrence rates increased proportionally with the grade of the varicocele. Comparative evaluation of the sperm parameters before and after surgery revealed 60% improvement with respect to sperm morphology, 41% improvement with respect to sperm density and 42% improvement with respect to that of motility.Conclusion: In the light of our findings we may conclude that antegrade sclerotherapy of the internal spermatic vein (Tauber operation) found to be effective, feasible and reliable from many aspects. Apart from acceptible surgical outcome, shorter operation time and decreased cost of the operation were the main causes to prefer this procedure. When it was compared with other techniques, it has a relatively better tolerance and demonstrates lower complication rates. We believe that this may be well applied in the surgical management of varicocele as an acceptable alternative.Keywords: Varicocele, SclerotherapyTurkiye Klinikleri J Med Sci 2002, 22:401-405
