Although the pathogenesis of acute rheumatic fever (ARF) completely understood, there are some points that remain obscure. The essential ingredient appears to be an induvidual with inherited immunological abnormalities encountering a strain of group A streptococcus (GAS) with rheumatogenic potential, possibly related to the presence of antigens cross-reactive with human tissue. Abnormal processing of GAS antigens and/or abnormal expression of self antigens result an exagerated cellular immune response with consequent damage to host tissue and the production of cross-reactive antibodies. Superantigens and copathogens may also play a role.
