The Effects Of Hyperbaric Oxygen And Antioxidantvitamin Combination On The Viable Area Ofexperimental Random-pattern Skin Flaps

The effect of antioxidant vitamins (E+C) and hyperbaric oxygen on random pattern skin flap which is a good model for flap studies was investigated. Focal necrosis and tissue failure have been encountered depending on the reactive oxygen species, after removing the flap. Hyperbaric oxygenation is a widely used method to decrease many ischaemic process. There are studies to show that vitamin E and C can be administered successfully in both ischemia-reperfusion injury and other pathologic situation characterized by reactive oxygen species production. In this study, caudal based random pattern dorsal flaps in the dimensions of 3x10 cm were raised from a total of 40 female Sprague-Dawley rats and separated into four equal groups as the control and the treated ones. In 7th day after flap raising, viable tissue area were evaluated as 64.9±2.23% for control, 53.7±1.89% for HBO only, 84.2±1.81% for vitamin only, and 66.8±1.55% for HBO+vitamin group. While viability ratios of vitamin only group increased significantly as compared with control ratios, that of HBO only group decreased (p<0.01 for both). Combination of HBO and vitamin showed no significant increase or decrease according to control group viability ratios (p>0.05). Although HBO has harmful effects leading to an increase in reactive oxygen species production on flap viability, further investigation with different HBO doses and time intervals is needed. The antioxidant vitamin E and C combination, however, seems to be effective for limiting necrosis ratios in flap surgery.
