Causes Of Delayed Diagnosis, Misdiagnosis Andproblem Of Perforation In Acute Appendicitis

In general surgery, acute appendicitis is the leading one among the diseases which are most frequently seen and require operation most commonly. The most important factor contributing on morbidity and mortality is delayed diagnosis.
In this study causes of delayed diagnosis and complications resulting from delayed diagnosis in acute appendicitisis analysed. 412 cases who were operated on due to the preliminary diagnosis of acute appendicitis were retrospectively surveyed. Perforation was detected in 114 (23%) of the patients. Forty (35%) of them were female, 74 (65%) were male. Mean age was 37 years (17-72).
The patients were responsible of the delayed diagnosis in 81 cases (71%), the physicians in 27 (23%) cases and both the patients and the physicians in 6 (6%) cases. The diseases which frequently caused misdiagnosis were as follows: urinary infection and urolithiasis (44%), pelvic inflammatory disease (30%), gastroenteritis (11.5%), the biliary tract diseases (11.5%), intestinal obstruction (3%). In the cases of perforated appendicitis, the mean duration between onset of complaints and operation was 4 days (range: 2-8 days).
In 33 of the cases with perforated appendicitis, various complications, of which the wound infection was the leading one, developed in the postoperative period. It can be concluded that early diagnosis of acute appendicitis presents an ease of treatment and very few- almost none- postoperative problems. And that delayed diagnosis cause serious problems and increased morbidity and mortality.
