Importance Of Erythrocyte Ferritin In The Of Iron Deficiency Anemia Seen Together Wİth Chronic Disease Anemia

Although serum ferritin is accepted as the most valuable and non-invasive test to determine total body iron store, it can also increase as one of the acute phase reactants in chronic diseases such as infection, inflamation or neoplasia; so it may lead mistakes in evaluating iron stores. It has been, in recent studies, reported that erythrocyte ferritin is able to reflect total body iron store without being affected by chronic diseases like serum ferritin. In this study clinical usefulness of serum and eryhrocyte ferritin levels were investigated in determining iron deficiency of CDA. 32 CDA subjects with high sedimentation (Hb<11.5 g/dl, MCV<95 fl) were included in the study. 16 cases who had no iron store demonstrated by iron stain in bone marrow aspiration were considered as iron deficiency anemia besides CDA.
There was a positive correlation between sedimentation and serum ferritin, but no relationship was found between sedimentation rate and erythrocyte ferritin. The relationship between grade of bone marrow prussian blue stain and erythrocyte ferritin was found to be more significant than serum ferritin. The diagnostic value and sensitivity of erythrocyte ferritin was higher, but spesificity was equal to those of serum ferritin in diagnosis of IDA.
As a result, this study showed that erythrocyte ferritin is more reliable than serum ferritin in diagnosis of CDA complicated with IDA, and sufficient alone for diagnosis of this situation.Keywords: Erythrocyte ferritin, Serum ferritin,
