Surgical Complications In Renal Transplant Patients

Incidence of surgical complications, methods used in their diagnosis, treatment alternatives, effects on graft function were evaluated in 133 patients who underwent renal transplantation because of chronic renal failure in Çukurova University Medical Faculty, Department of Urology between December 1989 and July 1999.
Sixteen surgical complications were encountered in 16 patients (12.0%). Seven (5.3%) had urinary fistulae, 5 (3.7%) had lymphoceles, 2 (1.5%) had renal arterial stenosis of less than 50%, 1 patient (0.7%) had ureterovesical stenosis and another one (0.7%) had hematoma. In the diagnosis, complete blood count, detailed serum analysis, urinalysis, renal ultrasound and/or scintigraphy were performed.
There was ureterovesical leak in 6 of the 7 patients with urinary fistulas; an anastomosis revision was performed on these. Pyelovesicostomy was done in 1 patient with distal ureteral necrosis. Ureteroneocystostomy was performed in 1 patient with ureterovesical stricture; external drainage or peritoneal marsupialisation was done in 5 patients with lymphoceles; hematoma drainage was performed in one patient with hematoma. Since the stenosis was not significant, no intervention was found necessary in 2 patients with renal artery stenosis. There was no graft function deterioration or graft loss caused by surgical complications.
In patients in whom surgical complications were seen after renal transplantation, the prompt diagnosis and successful management of the complications prevented the possible negative effects on graft function and life.
