Treatment Of Myelodysplastic Syndrome

** Uz.Dr., GATA Hematoloji BD, ANKARA In myelodysplastic syndrome, it hasn't been described an effective treatment except bone marrow transplantation to date. Therefore, the treatment consists of transfusion support in most cases. However, it was reported that immunosuppressive treatment was successful in hypocelluler myelodysplasia as in aplastic anemia. Differentiating agents are not usually effective in the treatment, but isotretinoin was effective in juvenile CML. The results of with 5-azacytidine that inhibits DNA methylation is promising. Amifostine that is a antiapoptotic agent has improved cytopenia in some patients. Erytropoietin may be effective to reduce the requirement of transfusion in patients with refractory anemia or patients with low level of serum erythropoietin. Myeloid growth factors may also be effective to increase neutrophil count in patients who have recurrent infections. There is a conflict on the results of treatment with one chemotherapeutic agent. Treatment with intensive chemotherapy regimens that are administered in AML have high mortality rate with 40-70 percent complete remission (CR). These regimens may be successful in high risk patients. Allogeneic bone marrow transplantation with high mortality rate seems to be the only curative treatment modality. Autologus peripheral stem cell transplantation in patients in CR following remission induction chemotherapy has been researched.
