Comparison Of The Specifity Of Ogtt And Euglycemic Clamp Test In Evaluation Of Insulin Resistance

** Dr., Ankara Numune Eğitim ve Araştırma Hastanesi CASS Bölümü, Objectives: To compare the specifity of determination of insulin resistance by the area under curves of blood glucose and plasma insulin levels during OGTT to euglycemic clamp test
Material and Methods: The study included 30 cases with hirsutism and 16 nonhirsute cases. 15 of the hirsutism group was overweight (OH overweight hirsute) and 15 was normoweight (NH normoweight hirsute), 8 of the nonhirsute group was overweight (ONK overweight nonhirsute) and 8 had normal weight (NNK normoweight nonhirsute). At 8 o'clock a.m., after 12 hours fasting, 75 g glucose was drunk by every case and blood glucose and plasma insulin levels were measured at the 30, 60, 90, 120, 180 minutes. Euglycemic clamp test was also performed in all cases.
Results: In the groups as a result of comparison of the area under the curve for OGTT values of plasma insulin levels, no significant difference was found between OH and NH (p>0.05), and the sum of OH were significantly higher than the sum of ONK and the sum of NNK (p<0.01), (p<0.001). When there was no significant difference between NH and ONK (p>0.05), the sum of NH was significantly higher than the sum of NNK (p<0.01). No significant difference was found between ONK and NNK (p>0.05). In the groups when M (g/U) walues were compared no significant difference was found between OH and NH (p>0.05), the M value of OH was significantly higher than the M values of ONK and NNK (p<0.001), (p<0.001). The M value of NH was also higher than the M values of ONK and NNK (p<0.001), (p<0.001). When compared ONK to NNK, the M value of ONK was significantly higher than the other (p>0.05).
Conclusion: We suggest that to determine insulin resistance by the area under curves of blood glucose and plasma insulin levels during OGTT can give misleading results, more sensitive OGTT methods must be preferred.Keywords: OGTT, Euglycemic Clamp Test, Insulin resistance,
