Hemodynamic Effects Of Rocuronium During Fentanyl - Midazolamanesthesia: Comparison With Vecuronium In Patients Undergoingcoronary Arter Bypass Surgery

*Doç.Dr., Ankara Üniversitesi Tıp Fakültesi Anesteziyoloji ve Reanimasyon AD, ANKARA In the present study, we aimed to compare the effects of vecuronium and rocuronium after midazolam - fentanyl induction in patients undergoing coronary artery bypass surgery (CABG). Following baseline recordings of hemodynamic variables (Awake), fentanyl 15 mgr/kg and 0.2 mg/kg midazolam were administered for the induction of anesthesia. Then, patients were randomly allocated to two groups. Group I (n=19) received vecuronium 0.1 mg/kg and group II (n=18) received rocuronium 0.7 mg/kg for muscle relaxation. Hemodynamic measurements were made prior to relaxant administration (T0), 120 seconds after the injection of muscle relaxants (T1) and tracheal intubation was facilitated afterwards. Then, all hemodynamic measurements were repeated 5 (T2), 10 (T3) and 15 minutes (T4) after endotracheal intubation. The administration of vecuronium was associated with decreases in heart rate (-10%), mean artery pressure (-15%) and central venous pressure (-18%) (p<0.05). In rocuronium group the changes in HR (+8%), MAP (-6%) and CVP (-11%) were insignificant. However, there were increases in stroke volume index (+18%) and cardiac index (+12%) following administration of rocuronium (p<0.05). The absolute values of all variables were within acceptable clinical limits. There was no evidence of histamine release in any patient. The present study shows that rocuronium 0.6 mg/kg is associated with changes of only small magnitude in hemodynamic variables.Keywords: Cardiac surgery, Hemodynamics,
